SureCircle does not believe in a one-size-fits-all solution. In fact, we’ve made our self-funded plans completely customizable so you can have exactly the right plan for your organization.  Below are some of the customizable options available:

SureCircle offers both traditional procedure-based and simple dollar-based plans. Procedure-based plans feature fully customizable treatment classes and in/out of network payout percentages and plan maximums. Dollar-based plans reimburse members based on actual money spent on treatment, regardless of treatment class.

Clients can choose between an open network where coverage is determined as a percentage of UCR fees, or they can opt for the DenteMax network with over 81,000 access points nationwide.

• Yearly Maximums:  Any amount client chooses
• Deductibles: $0 - any amount client chooses
• Percentage co-pay for in and out of network
• Out of network payout policy
• Restrictions, Limitations- As specified
